Medicare GLP-1 Bridge 2027: $50 Wegovy & Zepbound Coverage, Eligibility & How to Apply

Medicare is now covering certain GLP-1 weight-loss drugs for qualifying beneficiaries through a temporary national program called the Medicare GLP-1 Bridge.

The program began July 1, 2026, and CMS says it will continue through December 31, 2027.

Eligible Medicare beneficiaries pay $50 for a one-month supply of certain covered GLP-1 drugs.

Current covered products include:

  • Foundayo® tablet
  • Wegovy® injection
  • Wegovy® tablet
  • Zepbound® KwikPen®

But not everyone with Medicare qualifies. Eligibility depends on Medicare drug coverage, age, BMI, certain medical conditions, whether your Part D plan already covers a GLP-1 for your condition, and prior authorization.

What Is the Medicare GLP-1 Bridge?

The Medicare GLP-1 Bridge is a temporary CMS demonstration that provides certain Medicare Part D beneficiaries with access to selected GLP-1 drugs used for weight management.

CMS says the Bridge operates outside the normal Part D benefit payment flow. A separate national process handles prior authorization, claims processing and pharmacy payment.

CMS — Medicare GLP-1 Bridge

Why the Program Now Runs Through the End of 2027

CMS says the Bridge will remain available through December 31, 2027. The agency says the extension follows the decision not to launch the BALANCE Model in 2027 and allows additional GLP-1 utilization data to be collected.

Which GLP-1 Drugs Does Medicare Cover Through the Bridge?

Medicare currently lists Foundayo tablet, Wegovy injection, Wegovy tablet and Zepbound KwikPen.

The Bridge does not currently cover Zepbound single-dose vials or single-use pens.

How Much Does the Medicare GLP-1 Bridge Cost?

Eligible beneficiaries pay $50 per one-month supply at the pharmacy. A one-month supply is generally 28 or 30 days depending on the product.

The $50 Does Not Count Toward Your Part D Deductible

Because the Bridge operates outside the regular Part D benefit, the $50 copay does not count toward your Part D deductible.

For 2027, the defined-standard Part D deductible is $700.

See our Medicare Part D 2027 Costs guide.

The $50 Also Does Not Count Toward Your Part D Out-of-Pocket Threshold

The Bridge copay also does not count toward regular Part D True Out-of-Pocket spending. The 2027 defined-standard Part D out-of-pocket threshold is $2,400.

Extra Help Does Not Lower the $50 Bridge Copay

Medicare says Extra Help cannot reduce the Bridge’s $50 pharmacy copay.

The Medicare Prescription Payment Plan Does Not Apply

Medicare says the $50 Bridge copay cannot be spread over several months through the Medicare Prescription Payment Plan.

Who Is Eligible for Medicare GLP-1 Bridge Coverage?

You generally need to be at least 18, have qualifying Medicare drug coverage and meet one of Medicare’s BMI-and-health-condition pathways.

Pathway 1: BMI of 35 or Higher

If your BMI is 35 or higher, Medicare says you can satisfy the current BMI pathway without needing one of the additional listed conditions required at lower BMI levels.

Pathway 2: BMI of 30 or Higher Plus a Qualifying Condition

If your BMI is 30 to 34.9, Medicare currently requires at least one of these listed conditions:

  • diastolic heart failure, also called heart failure with preserved ejection fraction;
  • uncontrolled high blood pressure; or
  • chronic kidney disease stage 3a or higher.

Pathway 3: BMI of 27 or Higher Plus a Qualifying Condition

If your BMI is 27 to 29.9, Medicare currently requires at least one of the following:

  • prediabetes;
  • a previous heart attack;
  • a previous stroke; or
  • symptomatic peripheral artery disease.

Medicare GLP-1 Bridge Eligibility Table

BMIAdditional Requirement
35+No additional listed condition required under the BMI pathway
30–34.9Diastolic heart failure, uncontrolled hypertension, or CKD stage 3a+
27–29.9Prediabetes, prior heart attack/stroke, or symptomatic peripheral artery disease
Below 27Does not meet current Bridge BMI criteria

What Medicare Drug Coverage Qualifies?

Medicare lists coverage types including stand-alone Part D plans, Medicare Advantage plans with drug coverage, HMO/HMO-POS MA-PD plans, local and regional PPO MA-PD plans, Special Needs Plans, certain employer/union group plans and LI NET.

If you have Medicare but no qualifying drug coverage, you do not currently qualify for the Bridge.

What If You Have Type 2 Diabetes?

Medicare says people with type 2 diabetes are not eligible for the Bridge on that basis because their regular Part D plan may already cover GLP-1 therapy for diabetes.

What If You Have Sleep Apnea or Fatty Liver Disease?

Medicare similarly says moderate-to-severe sleep apnea and fatty liver disease can fall under regular Part D coverage pathways rather than the Bridge.

What If Your Part D Plan Already Covers Your GLP-1?

You generally cannot switch to the Bridge simply to get the $50 copay. Medicare says beneficiaries who already receive the drug through their Part D plan are not eligible for the Bridge for that therapy.

Do You Need Prior Authorization?

Prior authorization may be required. Your provider must prescribe a covered GLP-1 and, when requested, complete the authorization. The provider must also certify that the medication is being used as part of a lifestyle program focused on diet and exercise.

See our Medicare Advantage Prior Authorization 2027 guide.

How Long Does Bridge Prior Authorization Last?

Medicare says an approved Bridge prior authorization can remain valid through December 31, 2027, including refills and dose changes, unless you change GLP-1 drugs.

How to Apply for Medicare GLP-1 Bridge Coverage

  1. Confirm that you have qualifying Medicare drug coverage.
  2. Check whether your BMI and medical conditions meet Bridge criteria.
  3. Talk with your health-care provider.
  4. Your provider sends a prescription for a covered product.
  5. Your provider completes prior authorization when requested.
  6. Your provider certifies that treatment is part of a diet-and-exercise lifestyle program.
  7. You receive a coverage letter if approved.
  8. You pay the $50 monthly copay at the pharmacy.

Can You Check Eligibility Yourself?

Yes. Medicare.gov provides an interactive eligibility tool that asks about drug coverage, current GLP-1 use, health conditions, age and BMI.

Medicare.gov — Weight Loss Drug Coverage

Does the Program Cover All GLP-1 Drugs?

No. Do not assume Ozempic, Mounjaro or every formulation of Zepbound is covered through the Bridge. Some GLP-1 drugs may instead be covered by regular Part D for covered medical indications.

Wegovy Medicare Coverage in 2027

Wegovy is specifically included in the current Bridge list in both injection and tablet formulations, subject to the program’s eligibility and authorization requirements.

Zepbound Medicare Coverage in 2027

The Bridge currently covers Zepbound KwikPen. Medicare says single-dose vials and single-use pens are not included.

Does Medicare Cover Ozempic for Weight Loss Through the Bridge?

Medicare does not currently list Ozempic among Bridge-covered weight-loss products. Ozempic may have separate Part D coverage for an approved medical indication such as type 2 diabetes, depending on the plan.

What Happens If You Switch Medicare Advantage Plans?

The Bridge is a national CMS program and Part D sponsors do not have to opt in. However, you must continue to have qualifying Medicare drug coverage.

See our Medicare Open Enrollment 2027 Checklist.

What If Your Medicare Advantage Plan Is Ending?

If your MA-PD plan is being discontinued, make sure your replacement coverage includes Medicare drug coverage if you want to remain eligible for the Bridge.

See our Medicare Advantage Plan Ending in 2027 guide.

Does HMO vs PPO Matter for the Bridge?

Both qualifying HMO and PPO Medicare Advantage plans with drug coverage can support Bridge eligibility.

See our Medicare Advantage HMO vs PPO 2027 guide.

Does IRMAA Affect the $50 GLP-1 Copay?

IRMAA and the Bridge are separate. Higher-income beneficiaries may still owe Part B or Part D IRMAA, but Medicare currently lists the Bridge copay as $50 per monthly supply for eligible beneficiaries.

See our 2027 Medicare IRMAA Brackets guide.

Example: BMI 36 With Medicare Part D

A 67-year-old beneficiary with Part D and a BMI of 36 may satisfy the BMI pathway if the person also meets the program’s other requirements and is approved through the prescription and authorization process.

Example: BMI 31 With Chronic Kidney Disease

A beneficiary with BMI 31 and chronic kidney disease stage 3a meets one of Medicare’s current BMI 30+ condition pathways, subject to the other Bridge requirements.

Example: BMI 28 With Prediabetes

A beneficiary with BMI 28 and prediabetes fits one of the current BMI 27+ condition pathways, again subject to all other requirements.

What If You Are Already Taking a Weight-Loss GLP-1?

Current use does not automatically determine eligibility. Medicare still considers your drug coverage, BMI, conditions, product and whether Part D is already paying for your therapy.

Does the Bridge Continue in 2028?

CMS currently says the Bridge ends on December 31, 2027. Beneficiaries should not assume that the $50 copay or the same eligibility rules will continue into 2028.
