2027 Retirement Guide: Social Security, Medicare, COLA & Major Changes
Retirees are heading into 2027 with several important Social Security and Medicare changes to track.
Some 2027 numbers are already final. Others are still projections and should not be treated as official yet.
For example, the Centers for Medicare & Medicaid Services has already finalized the $700 defined-standard Medicare Part D deductible and $2,400 Part D out-of-pocket threshold for 2027.
CMS has also published 2027 Medigap limits, Medicare Advantage and Part D market projections, and negotiated prices for 15 high-cost Part D drugs.
But as of October 7, 2026, the Social Security Administration has not yet announced the official 2027 COLA. The final September CPI-W reading needed for the calculation is scheduled for release on October 14.
The final 2027 Medicare Part B premium is also not yet official. The Medicare Trustees currently project a standard monthly premium of $209.50, but CMS has not finalized that amount.

1. Social Security COLA for 2027: What We Know Now
The official 2027 Social Security cost-of-living adjustment has not yet been announced.
Social Security says the next COLA will be announced in October 2026.
The calculation uses the Consumer Price Index for Urban Wage Earners and Clerical Workers, or CPI-W, during July, August and September.
Two of the three 2026 readings are already known:

The September CPI report is scheduled for October 14, 2026. That release should provide the final CPI-W input needed to calculate the 2027 COLA.
Current forecasts generally cluster around approximately 3.4% to 3.6%, but those percentages remain projections until SSA announces the official adjustment.

2. Estimate Your 2027 Social Security Check
Until the official COLA is published, retirees can use a range rather than assuming one exact percentage.
For example, if someone currently receives $2,000 per month:

3. Full Retirement Age Reaches 67 for People Born in 1960 or Later
One Social Security rule that is already established is full retirement age.
For people born in 1960 or later, full retirement age is:

That means people born in 1960 reach their full retirement age during 2027.
Retirement benefits can still begin as early as age 62, but starting before full retirement age generally reduces the monthly benefit.
Waiting beyond full retirement age can increase retirement benefits through delayed retirement credits until age 70.

4. Medicare Part B: The 2027 Premium Is Still a Projection
The standard Medicare Part B premium is:
$202.90 per month in 2026
The 2026 Medicare Trustees Report currently projects a 2027 standard premium of:
$209.50 per month
That would be an increase of:
$209.50 − $202.90 = $6.60 per month
or:
$79.20 over 12 months
But $209.50 is not yet the final 2027 Part B premium.
The Trustees also project a Part B annual deductible of:
$292 in 2027
compared with the official 2026 deductible of $283.
The projected increase would be:
$292 − $283 = $9
Again, the $292 amount should be treated as a projection until CMS publishes the official 2027 figure.

5. Why Medicare Can Reduce the Increase You See in Your Social Security Deposit
Many Social Security beneficiaries have their Medicare Part B premium deducted directly from their monthly benefit.
That creates a difference between:
gross Social Security increase
and:
net increase after Medicare deductions
For illustration only, suppose:
- the final COLA were 3.5%; and
- the projected $209.50 standard Part B premium became final.
A person receiving a $2,000 benefit would have a simplified gross COLA increase of:
$2,000 × 3.5% = $70
The additional projected Part B premium would be:
$6.60
Approximate increase remaining after only that additional Part B cost:
$70 − $6.60 = $63.40 per month
This is only a planning illustration. Actual Social Security payments and Medicare deductions depend on individual circumstances.
6. Medicare Part D Deductible Rises to $700 in 2027
Unlike the Social Security COLA and Part B premium, the 2027 defined-standard Medicare Part D benefit parameters are already final.
CMS set the 2027 Part D defined-standard deductible at:
$700
The 2026 amount is:
$615
Increase:
$700 − $615 = $85
Percentage increase:
$85 ÷ $615 × 100 ≈ 13.8%
Individual Part D plans can use different benefit designs within Medicare rules, so a specific plan’s deductible may differ.

7. Part D Out-of-Pocket Threshold Rises to $2,400
CMS also set the defined-standard annual Part D out-of-pocket threshold at:
$2,400 in 2027
The 2026 threshold is:
$2,100
Increase:
$300
Under the redesigned Part D structure, beneficiaries owe no cost sharing for covered Part D drugs during the catastrophic phase after reaching the applicable threshold.
8. Medicare Advantage Premiums Are Projected to Fall
CMS projects the weighted average monthly premium across Medicare Advantage plans to decline from:
$14.37 in 2026
to:
$12.00 in 2027
That is a projected decline of approximately:
16.5%
CMS also says more than 99% of Medicare beneficiaries are expected to have access to at least one Medicare Advantage plan in 2027, while 97% are expected to have access to 10 or more options.
National averages do not tell you whether your own plan is cheaper or better in 2027.
Provider networks, copayments, deductibles, drug formularies and supplemental benefits can all change.

9. Stand-Alone Part D Premiums Are Projected Around $36
CMS projects the average monthly premium for stand-alone Part D prescription drug plans at approximately:
$36 in 2027
compared with:
$35.09 in 2026
That is an increase of less than $1 in the national average.
For Medicare Advantage plans that include prescription drug coverage, CMS projects the average Part D premium component to fall from:
$11.32 in 2026
to:
$7 in 2027
or approximately:
38% lower
Actual premiums vary by plan and location.
10. Fifteen Medicare Drug Prices Change in 2027
Another major 2027 development is Medicare’s negotiated prices for 15 selected high-cost Part D drugs.
The prices take effect:
January 1, 2027
The selected drug groups include medications such as:
- Ozempic, Rybelsus and Wegovy;
- Trelegy Ellipta;
- Linzess;
- Tradjenta;
- Janumet;
- Breo Ellipta;
- Xtandi;
- Ibrance;
- Calquence; and
- others.
CMS estimates that approximately 5.3 million Part D beneficiaries used the selected drugs in 2024.
The agency estimates the negotiated prices could save beneficiaries approximately $685 million in collective out-of-pocket costs in 2027 under the defined-standard benefit design.
Importantly, a negotiated price is not automatically the amount a beneficiary personally pays at the pharmacy.
Actual cost sharing depends on the Part D plan, deductible, formulary, pharmacy and other benefit rules.

11. Medigap High-Deductible Plan G Rises to $3,050
CMS has finalized the 2027 high-deductible amount for Medigap Plans F, G and applicable older Plan J policies at:
$3,050
The 2026 amount is:
$2,950
Increase:
$100
The $3,050 figure applies specifically to the high-deductible versions of those plans. It is not a new deductible for every standard Plan G policy.

12. Medigap Plan K and Plan L Limits Increase
CMS also finalized the 2027 out-of-pocket limits for Plans K and L:
Plan K: $8,360
Plan L: $4,180
Those compare with 2026 limits of $8,000 for Plan K and $4,000 for Plan L.
Beneficiaries comparing standardized Medigap coverage should remember that premiums are set separately by private insurers and can vary widely.
13. Plan G vs. Plan N: A Major Medigap Decision
Plan G and Plan N cover many of the same Original Medicare cost-sharing gaps, but they differ in important ways.
Plan G covers Medicare Part B excess charges.
Plan N does not.
Plan N can also require:
up to a $20 copay for some office visits
and:
up to a $50 ER copay when the visit does not result in an inpatient admission
Premiums vary by insurer and location, so the lower-premium option is not automatically the lower-cost choice over an entire year.

14. Medicare Open Enrollment Runs October 15 Through December 7
Medicare Open Enrollment for 2027 coverage runs:
October 15 through December 7, 2026
During this period, depending on your current coverage, Medicare beneficiaries can generally:
- switch Medicare Advantage plans;
- move from Original Medicare to Medicare Advantage;
- leave Medicare Advantage and return to Original Medicare;
- join or switch Part D drug plans; and
- make other permitted plan changes.
Changes generally take effect January 1, 2027 when the plan receives the enrollment request by the deadline.

15. What Retirees Should Check Before 2027
A retirement plan for 2027 should consider more than one headline number.
Before the year begins, review:
- your official Social Security COLA notice when available;
- your Medicare Part B premium;
- your Medicare Advantage or Part D Annual Notice of Change;
- prescription drug formularies;
- preferred pharmacies;
- doctor and hospital networks;
- Medigap premium changes;
- your expected out-of-pocket medical spending; and
- your overall retirement budget after inflation.
The biggest mistake is assuming that a higher Social Security benefit automatically means an equal increase in spendable income.
Health-care premiums, prescription costs, taxes and personal inflation can all affect the amount you actually keep.
2027 Retirement Planning Example
Consider a retiree receiving a $2,500 gross monthly Social Security benefit.
If the final COLA were 3.5%:
$2,500 × 0.035 = $87.50
Simplified gross new benefit:
$2,587.50
If the standard Part B premium then increased by the Trustees’ projected $6.60:
$87.50 − $6.60 = $80.90
That would leave approximately $80.90 of the monthly gross increase after accounting only for the additional standard Part B premium.
This example does not include Part D premiums, Medicare Advantage premiums, Medigap premiums, IRMAA, taxes or other deductions.
